If you are experiencing an issue with your Studio, please contact our Member Support team (support@formelife.com) so that we can help you troubleshoot.
Once we help to diagnose your issue related to accidental damage, and determine that it will require a replacement, we’ll guide you through submitting a claim through our Accident Protection Plan partner, Mulberry.
To submit a claim:
- Visit getmulberry.com/login and sign in using your credentials.
- Select the protection plan that you would like to file a claim under.
- Upload the following information regarding your claim into the chat:
- Incident date
- Photos of the damage
- Incident description
- The claim will be reviewed within 5 business days, and the FORME Member Support team will be in touch with you to set up your replacement order.
