How do I submit a claim?

  1. If you are experiencing an issue with your Studio, please contact our Member Support team ( so that we can help you troubleshoot. 

    Once we help to diagnose your issue related to accidental damage, and determine that it will require a replacement, we’ll guide you through submitting a claim through our Accident Protection Plan partner, Mulberry.

    To submit a claim:

    1. Visit and sign in using your credentials.
    2. Select the protection plan that you would like to file a claim under.
    3. Upload the following information regarding your claim into the chat:
      1. Incident date
      2. Photos of the damage
      3. Incident description
    4. The claim will be reviewed within 5 business days, and the FORME Member Support team will be in touch with you to set up your replacement order.
